What is Sildenafil?

The first successful oral ED drug was sildenafil. Vardenafil (Levitra) and tadalafil joined it in 2003. (Cialis). These medications all work exactly the same way to treat ED, despite some differences (tadalafil works longer and vardenafil starts working sooner).

The three medications are similar because they all target the same enzyme, phosphodiesterase-5 (PDE-5). However, you must be familiar with the process of erection development to comprehend why blocking PDE-5 enhances sexual function.

A receptive mindset, adequate testosterone levels, and healthy arteries, veins, and nerves are all necessary for normal erections. However, they also need a very small chemical messenger known as nitric oxide (NO). Because it instructs the arterial cells to produce cyclic guanosine monophosphate (cGMP), the chemical that increases blood flow to the penis, nitric oxide is crucial. But PDE-5, the enzyme that degrades cGMP, is also produced in the penis’ tissues.

The penis normally produces enough cGMP to cause a rigid erection and enough PDE-5 to cause the erection to break after ejaculation. This complex system is frequently out of balance in men with ED, but one of the three oral medications can frequently restore it. They increase the amount of cGMP available by inhibiting PDE-5, which for many men will enable the development of erections.

Vardenafil and tadalafil inhibit PDE-5 slightly more potently in test tubes than does sildenafil. But clinical activity is not always predicted by laboratory potency. In this case, it means that lower doses of the newer medications will produce outcomes that are comparable to the success rate of the older medications.


Vardenafil is sold in tablets with doses of 2.5, 5, 10, and 20 mg, tadalafil in doses of 5, 10, and 20, and sildenafil in doses of 25, 50, and 100 mg. Each is more effective at higher doses, but the highest doses also carry a greater risk of side effects. Additionally, all medications work better in men with mild ED than in those with severe impairment.

With sildenafil, doctors have more clinical expertise. However, the effects of all three drugs appear to be comparable. Approximately 70% of men benefit, in general. The response is most favorable in men without a clear organic cause of ED (about 90%), less so in diabetics (about 50%), and difficult to predict in men receiving treatment for prostate cancer.

Side Effects

The side effects of these medications would only affect the penis if PDE-5 were only present in that obdurate organ; in fact, priapism, which causes painful prolonged erections that call for immediate treatment, is a rare side effect. PDE-5, however, is also present in trace amounts in blood vessels throughout the body. PDE-5 is just one of the 11 enzymes that make up the phosphodiesterase family. The body’s various regions are where the others are concentrated. Although PDE-5 is the PDE for which ED medications have a much stronger affinity than the other PDEs, they do have some capacity to inhibit those closely related enzymes. Both of these factors help to explain why ED medications occasionally cause side effects in various body regions.

Headaches and flushing of the face are the most frequent side effects, occurring in 15% of men. Back pain, indigestion, and nasal congestion are additional side effects; blue-tinged vision is even less frequent. These negative effects are typically minor and short-lived. However, fresh data has expanded the list to include a rare eye condition. Blindness may result from a condition known as nonarteritic anterior ischemic optic neuropathy (NAION), which is poorly understood. Seven men experienced visual impairment within hours of taking sildenafil, according to medical reports from March 2005. Since then, more instances involving sildenafil, tadalafil, and vardenafil have been reported to the FDA. Thankfully, there are very few cases compared to the millions of men who have used ED medications successfully.

The cause-and-effect relationship between ED medications and NAION is not entirely clear. Although there is currently not enough cause for concern to stop men from using these pills, all men should still use them responsibly and cautiously.
